HOW DO I....
Obtain A Repeat Prescription?
48 hours’ notice is required for repeat prescriptions. We have
a computerised system for patients who require regular supplies of medication. A
new list of medication is issued with each prescription - this must be brought
to the Health Centre (or posted to us) when requesting a prescription. This ensures
that all prescriptions are correctly written and automatically checked against
your records. If your medication has changed or you are requesting it early,
eg for holidays, then please make this clear on your request slip.
If you do not have an order form then please write down your name, address
and full details of the medication you require.
All written requests should be put in the box provided at reception.
If you wish to chemist to deliver your medication then please contact them
direct
NB: Requests for repeat medication will not be taken over the telephone.
Sickness Certificates
A sickness certificate completed and signed by your doctor is only required
when a period of continuous absence from work, due to illness, is one of seven
days' duration or longer. For absence up to three days no certificate is
required. Between three and six days' absence is adequately covered by
a ‘self-certificate’ (available from any employer). All employers
should be aware of these regulations.
If your employer still requires a certificate for an absence of less than
seven days we can supply a private certificate for which a fee may be charged.
